OYUNA HOSPICE IS HIRING! Oyuna is a fast-growing and locally owned organization with an excellent reputation throughout the central valley. We are constantly growing and now seeking a Full time RN on-call to join our team.
As a member of the Interdisciplinary Team the Hospice RN is an experienced nurse who provides skilled end-of-life care in the community setting. The Hospice RN is responsible for promoting the Hospice philosophy. This individual facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration, and medical management of hospice services for patients and their significant others.
QUALIFICATION AND REQUIREMENTS
- At least 12 months of RN experience preferred
- Completion of a professional accredited nursing program
- Current RN License,
- Current CPR American heart association
- Must have a valid driver’s license and a willingness to use personal vehicle at times in the course of employment.
- Automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36-month period).
- Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.)
- Provides on-call coverage as scheduled and consistent with Oyuna Hospice policies and procedures.
- Communicates with attending physicians, Oyuna hsopspice staff members, and other agencies as necessary to coordinate optimal care and use of resources for patients/caregivers.
- Educates patients/caregivers regarding hospice philosophy, disease progression, physical components of care, stages of dying, and other aspects of the POC as necessary.
- Reports changes in patients/caregivers status to IDT so that problems, plans, actions and goals are accurately and clearly documented and changes are reflected as they occur. Documents all information according to Oyuna Hospice policies and procedures.
- Responds to medical or emotional crisis and/or death of patient by visits to home/facility to provide direct care as appropriate.
- Responds to patient/caregiver phone calls in a timely manner consistent with Oyuna Hospice policies and procedures.
- Communicates report to the IDT following each shift.
- Exhibits cost effective use of time, mileage and supplies.
- Understands and exemplifies hospice philosophy, values, beliefs and team concepts in all job-related roles.
