Job Summary:
Under the general supervision of the Clinic Services Director, the Clinic RN is responsible for operations within the medical clinic. The Clinic Registered Nurse is expected to utilize a systematic approach to nursing practice which incorporates all aspects of the nursing process including assessment, planning, implementation, and evaluation of PACE Participants. The Clinic RN’s documentation provides evidence of a direct relationship between nursing interventions and participant outcomes, demonstrates clinical competence, and provides effective patient teaching for acute and chronic illnesses. The focus of care is one that enhances optimal quality of life, provides for dignity in all aspects of care, and encourages Participant, Caregiver and Family involvement in achieving health care goals.
Qualifications:
Education: BSN Preferred.
Licensure/Registry/ Certification: Registered Nurse Licensure required; CPR certification required
Experience: At least one year working with the frail elderly population.
Skills:
- Must possess the skills necessary to provide care for geriatric patients.
- Ability to think and work independently and with direction and communicate with staff members in a fast-paced and sometimes stressful environment.
- Ability to function well in a team environment.
- Possess basic computing skills in the workplace.
- Ability to document in the electronic medical record.
- Act within the authorized scope of practice.
- Ability to meet a standardized set of competencies for the specific position description established by PACE North and approved by CMS before working independently.
- Maintain confidentiality.
- Must possess critical thinking skills to ensure best practice.
Immunizations: Be medically cleared for communicable diseases and have recommended immunizations up to date before engaging in direct contact with participants.
Responsibilities:
Clinic:
- Attends meetings as necessary to serve as a resource to other disciplines and to facilitate communication with clinic staff regarding participant concerns.
- Gathers assessment data and documents appropriately in electronic medical records.
- Performs interventions within the RN scope of practice based and as ordered by primary care providers.
- Evaluates effectiveness of interventions and documents, providing evidence of a direct relationship between assessment, interventions, and effectiveness of care.
- Collaborates with IDT (Interdisciplinary Team) members to ensure participant needs are addressed.
- Completes all necessary admission paperwork and is present during care plan meetings to provide input.
- Provides on-call support as scheduled.
- Answers incoming calls to the clinic, triaging care needs expressed by the caller, and taking appropriate action or giving instructions for next steps.
- Collects data and participates in quality improvement projects.
- Provides information and tracking for ongoing regulatory requirements.
- Assists with ordering of supplies.
- Maintains required certifications, annual competencies, and education credits as per policy.
Day Center:
- Provides nursing care for participants in the day center, including medication administration and dressing changes as ordered.
- Documents provision of care in the electronic medical records.
- Responds to care needs expressed by participants in the day center or communicated by the Day Center Staff.
- Performs assessments and triages care needs.
- Collaborates with physicians and nurse practitioners to ensure reported needs of participants are met.
- Collaborates with Day Center Staff to ensure participants’ care needs are met.
- Performs rounding/is present in the day center frequently throughout the day.
Working Conditions:
- The environment may include noise, frequent interruptions, possible exposure to unpleasant odors including odorous wounds, urine, feces, other excrements, cleaning products, chemicals, and disinfectants.
- Work requires standing, walking, lifting, bending, and sitting.
- Flexibility to support participants in both the Primary Care Clinic and Day Center. Occasional travel into our service areas should be anticipated.
- Potentially dangerous equipment may be utilized and/or maintained.
- Prescribed dress code will be according to the PACE North policy.
- Manual dexterity is required to complete tasks; walking, stooping, standing, reaching, bending and lifting of forty (40) pounds or more may be needed in the performance of duties.

Salary : $61,600 - $78,000
