Job Title: Health Care Case Manager
Job Description: Provide superior customer service and quality home care. Manage coordination of client services and emergent scheduling issues. Manage caseload. Build lasting relationships with clients, referral sources, payors and community organizations. Review individual care plan and oversee its execution.
Minimum Requirements: Bachelor degree in Nursing or related. Knowledge of or experience in assessing patient health problems, implementing nursing care plans, and advising patients on health maintenance. Verifiable references. Able to work overtime, including late nights, weekends, and holidays. Non-smoker on worksite.
