What are the responsibilities and job description for the Clinical Intake LVN position at Palomar Health?
Under the direct supervision of the Home Health Nurse Manager, the Clinical Intake LVN is responsible for the coordination of Home Health services. Responsibilities include but are not limited to the following: coordinating discharge planning for patients in the Palomar Health system with other area hospitals, Skilled Nursing Facilities, Assisted Living Facilities, and physician offices. Collaborates with patients, families, discharge planners, physicians, case managers, and other members of the health care team to assure a smooth discharge transition to Home Health services. Acts as liaison with third party payers, physicians, patient families and the community to ensure continuum and quality of care. Participates in assessment of patient needs and support systems and the appropriateness of all referrals under applicable laws and regulations. Educates referral sources regarding admission criteria for the various Home Health programs.
Ability to speak and read English at a level that is sufficient to satisfactorily perform the essential functions of the position. Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Access, Word, PowerPoint, internet, e-mail, Write Fax). Computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job. Performs other duties as assigned as it relates to Home Health. Follows Palomar Health rules, policies, procedures, applicable laws and standards. Carries out the mission, vision, and quality commitment of Palomar Health.
