Position Details:

• Full Time Social Work Position
• Territory: Fort Wayne and surrounding communities

Experience:

• One year Hospice experience is preferred, but not required
• One year of healthcare experience is required
• MSW required at a minimum

Credentials:

• Licensed Social Worker in State of Indiana
• Current driver's license
• Must have reliable transportation and insurance
