Job Summary and Qualifications
As an Advance Care Planning Facilitator, your primary responsibility is to ensure that an effective program of advance care planning is integrated into the HDH (HCA VA Richmond Market) such that the advance care planning health needs of patients, families, and HCA team members are assessed and met in accordance with advance care planning best practices as established by the Honoring Choices Virginia (Respecting Choices) and the Virginia POST (Physician Orders for Scope of Treatment) paradigms. Additional responsibilities include:
- Design and implement educational programs for health care professionals to improve their understanding of advance care planning to include Advance Directives, Durable Do Not Resuscitate and POST.
- Design and disseminate a mechanism for referral to qualified HDH/HCA advance care planning facilitators.
- Assist with the development and implementation of organizational systems that support HDH/HCA advance care planning objectives including policies and protocols
- Work in close collaboration with key HDH/HCA programs and departments to develop and promote quality advance care planning including Palliative Care, Physicians, Nursing, and others.
- Provide support to patients, families, and HDH/HCA team members struggling with end of life decisions.
- Promote Advance Care Planning educational events to include, but not limited to National Healthcare Decisions Day.
- Assist with Ethics, End of Life, Organ Donation support and education.
- Provide oversight to the HCV data collection associated with patient accruals, process changes, and documentation issues to assess results obtained and make improvements as needed.
What qualifications you will need:
- Active Registered Nurse license in the state of Virginia
- Certified Respecting Choices Advance Care Planning facilitator and instructor
- Certified Respecting Choices Next and Last Steps and Virginia POST Collaborative advance care planning facilitator and instructor (completed within 1 year of hire)
- Critical Care or Hospice experience required
- Familiarity and experience with healthcare advance directives required
- Excellent written, oral communications, conflict resolution and interpersonal skills
- Experience with public speaking
- Proficiency with Meditech, hCare Portal, HPF, Excel, and PowerPoint
