What are the responsibilities and job description for the MDS Coordinator position at Parkway Health?
Parkway Health and Rehab, L.L.C., a skilled long-term care facility located in Kansas City, Missouri, has an opportunity for an experienced MDS Coordinator. This position will involve management of the MDS processes in accordance with government regulations and company policies, and the coordination of the care plan process with focus on maintaining excellent care for the residents. Requirements include an unencumbered Missouri RN or LPN license with at least 2- years' experience in long-term care, knowledge of Medicare regulations, experience using MDS 3.0 and a commitment to Total Quality Management. Must be a team player!
Duties Include (but are not limited to)
- Process and transmit all MDS documents.
- Assist with dinning needs.
- Chart / Document resident care plans
- Engage / Converse / Have Fun with residents.
- Periodically assist to cover a shift
