JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Perform liaison duties for medical department; maintain medical supplies; coordinate patient assistance process; obtain prior authorizations for medications, complete treatment plans, provide Clozaril monitoring and evaluation, medication administration and evaluation and interface with physical health providers to ensure integrated care and other assigned duties to assist physicians in meeting consumer needs. On behalf of individuals with BH/ID diagnosis, provide liaison services with psychiatrist, attend psychiatric visits and follow-up with psychiatric orders on behalf of individuals needs.
QUALIFICATIONS:
- Active Pennsylvania Registered Nursing License
- At least 1 year of experience in the provision of nursing care
- Good organizational skills
- COVID-19 Vaccination, or willingness to receive vaccine required.
SPECIFIC DUTIES:
- Work in conjunction with PATH’s medical department to provide ancillary services as requested by psychiatrist
- Call medication into pharmacy as directed by physicians
- Call medical specialists, residential programs, etc. as directed by physicians
- Maintain medical supplies and medications, ensuring that expired medications are disposed per Path policy.
- Meet with pharmaceutical representative as assigned
- Coordinate and monitor patient assistance program as assigned
- Obtain prior authorization for medications as assigned by physicians
- Distribute labeled medications as assigned by physicians
- Evaluate and assess the needs of individuals on caseload who are diagnosed with behavioral health and intellectual disabilities
- Coordinate efforts directed toward meeting the needs of individuals on caseload
- Provide liaison services as follow-up to psychiatric visits as requested by psychiatrist
- Work closely with psychiatrist and other medical professionals supporting the individual’s integrated health care
- Coordinate all case management functions and service activities as discussed with PATH psychiatrist
- Monitor individuals progress in day and/or residential programs and maintain communications with representatives from those programs
- Participate in psychiatric appointments for individuals on caseload
- Be aware of individual’s financial liabilities, coordinate any financial changes with the Billing Department, and assist individuals in securing third party resources when indicated
- Be familiar with administration of LAI, Narcan and obtaining UDS, utilizing breathalyzer, Oral swabs and other diagnostic tools.
- Provide Clozapine Monitoring and cooperate with other ancillary services as needed .
- Obtain supplies from pharmacy as needed.
- Keep strict account of number of medications ordered and administered for control substances.
