Pathways Medical Rehabilitation Program offers patients an alternative to an institutional short-term rehabilitation. We’re looking for compassionate full-time RN's to join our team!
Main Office Location: York, ME
Territory: Dover, NH and Surrounding Areas
Job type: Full-time, Monday-Friday, 40 hours/week, 8:30-5:00

Registered Nurse (RN) Responsibilities:
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es)
- Initiates the plan of care and makes necessary revisions as patient status and needs change
- Collaborate with physicians, nurse practitioners and interdisciplinary team to create a strong Plan of Care for each patient
- Respond to inquiries and concerns from patients, their caregivers and their medical team
- Counsel patients and caregivers before, during and after treatment
- Supervise home health aides as needed
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient
Registered Nurse (RN) Skills:
- 2 years’ nursing experience, preferably in geriatrics
- Bachelor’s Degree in Nursing
- Proficient with electronic medical records
- Excellent bedside manner and communication skills
- Working knowledge of health care laws and regulations
