Job Summary: Under the direction of the Senior Vice President of Health Center Programs, the Health Equity Specialist will coordinate efforts to address the root causes of health inequities through the assessment, planning, implementation and evaluation process. The Health Equity Specialist will further build internal organizational capacity; engage community partners; develop partnerships and coalitions; seek appropriate grant funding; identify and analyze health inequities in collaboration with the Quality Assessment and Planning Teams; select, design and implement strategies; develop effective communication efforts; and conduct evaluations.
The goal of health equity integration is to ensure person-focused care that includes individuals, families and communities from diverse backgrounds, and culturally competent strategies that address social determinant of health, health literacy, accessibility and inclusiveness.
Responsibilities:
Education/Experience:
Education: Requires a bachelor's degree in public health, community health education, social work or related field.
Experience: Two or more years of relevant experience required. Experience in GIS software applications preferred.
Computer Skills: Intermediate/Advanced - ability to access the intra/internet to manage timecard, review policies and procedures, and read company communications; use e-mail to communicate with co-workers, leadership, and other departments; use and create a variety of templates, complex tables, merges; manage table data, sort and filter merges, and also perform basic work with existing Macros; customize toolbars, import and insert graphs, embed Excel data, and elaborate reports; work with multiple worksheets, filter data, use integrate functions, and manipulate databases; customize templates and the PowerPoint environment, and to make a presentation interactive by using hyperlinks and action buttons.

Responsibilities:
- Conducts community need assessments and analyses to identify unmet needs and barriers to achieving health equity for all individuals, to include HRSA mandated need assessments, community profiles to support planning and program development initiatives, and need assessments to support Change in Scope applications and planning initiatives.
- Performs geo-spatial mapping, utilizing UDS Mapper and ARC/GIS software applications to support assessment and planning efforts.
- Develops and maintains collaborative partnerships internally and with other community organizations that address the same or related service population(s).
- Participates in performance management and monitors and reports on goals, objectives, outputs and outcomes.
- Collaborates with the Quality Team and Care Coordination to identify and impact Social Determinants of Health (SDOH) for Peak Vista patients.
- Collaborates with public health and other community partners to identify and address SDOH for communities within Peak Vista's service area.
- Develops and delivers in-service trainings to PV staff on health equity and the social determinants of health (SDH).
- Conducts need assessments to support grant applications and Change in Scope submissions.
- Performs an annual review of the Peak Vista Service Area and recommends changes to the Service Area boundaries as needed.
- Develops and maintains computer skills necessary to complete work.
- Assures that staffs receive/maintain necessary training and certifications in order to performed assigned prevention activities and interventions.
- Researches and supports evidence-based strategies to address health inequities.
- Supports and initiates coalition development to further build capacity to increase health equity throughout the Peak Vista service area.
- Promotes and supports development of local and state-wide policies related to increasing health equity.
- Collaborates with local Health Departments, supporting analytical studies, tracking and research projects as appropriate.
- Strengthens and supports community efforts to coalesce and form strategic alliances to address Health disparities and economic inequities.
- Promotes and demonstrates the mission, vision, and Core Values of Peak Vista.
- Develops and maintains good working relationships with all departments, the management team, medical staff, and leadership.
