What are the responsibilities and job description for the Transitional Care Nurse position at Pediatric Home Service?
We are passionate about helping kids thrive at home and are looking for a Transitional Care Nurse who shares that passion to help transition medically-fragile children from the in-patient to home setting.
THE POSITION
- Location: Omaha, NE
- Title: Transitional Care Nurse
- Schedule: Full Time
- Shift: Flexible to work a variety of days and shifts around the Omaha metro area, to support the unique needs of patients transitioning to the home setting. When possible, schedules will be created 2 weeks in advance. Differentials are available for night and weekend shifts.
- position will support a patient and family as they transition to the home setting for the first time, after a hospitalization or new diagnosis/technology. The Transitional Care Nurse ensures the journey from hospital to home is a smooth process, starting in the hospital with bedside training to being settled at home and ready to hand off cares to the family and long-term nursing team. The Transitional Care Nurse helps the patient and family be comfortable and confident at home after discharge.
EXPERIENCE & CERTIFICIATIONS:
- Must be a Registered Nurse licensed in good standing with Nebraska state board of nursing.
- Two years of experience working as a RN, pediatric experience required.
- ICU or other critical care level experience preferred.
- Experience training or providing education to nurses is preferred.
