Position Title: Project Lead
Project: Field Services Program
GENERAL DUTIES:
The Field Services Program (FSP) is responsible for linking newly diagnosed persons living with HIV (PLWH) to care, reengaging PLWH who are out-of-care and following up on other identified HIV cases. This includes the collection and analysis of participant data. The Project Lead will direct the FSP and is responsible for oversight of the FSP supervisor. This full-time position is in the Philadelphia Department of Public Health (PDPH), AIDS Activities Coordinating Office (AACO). This position is supervised by the Program Implementation Administrator in coordination with the AACO Director. The job entails updating and maintaining Standard Operating Procedures and developing protocols for linkage to care of newly diagnosed HIV-positive persons and field activities in response to HIV outbreaks and cluster investigations. The position includes extensive coordination with various Units and teams within AACO, including, but not limited, to the Outbreak Response Team, the Surveillance, Information and Client Services Units as well as other pertinent divisions within PDPH. This employee will also be expected to work collaboratively with AACO funded providers to engage out-of-care and newly diagnosed PLWH in HIV medical care.
RESPONSIBILITIES:
- Responsible for the maintenance of the Field Services Program including updating policies, standards and procedures annually.
- Lead efforts to transition existing procedures, protocols, data collection and reporting to meet the PA Department of Health’s program and data requirements for Outreach Services.
- Develop and implement the protocols for linking newly diagnosed individuals to HIV medical care and investigating sentinel cases.
- Ongoing assessment of staffing needs.
- Manages and ensures adequate data collection through the updating and revising of comprehensive data forms and oversight of programmatic databases.
- Works in collaboration with the Lead Epidemiologist and the epidemiologists assigned to this program for data collection, management and analysis.
- Supervise FSP supervisor.
- Provides technical assistance to and training of field staff in collaboration with the supervisor.
- Oversight of the evaluation of program processes and protocols making changes as needed to enhance efficiency and effectiveness in coordination with the Information Services and Surveillance Units.
- Works with AACO Management team to evaluate the FSP and develop alternatives and revisions to meet emerging needs.
- Prepares periodic reports on program achievements and progress towards program goals for the AACO Program Implementation Administrator and/or the Director.
- Prepares annual reports to the funder to describe overall program accomplishments.
- Collaborates with the PA Department of Health and provide guidance on procedures and best practices for supporting statewide outreach initiatives.
- Develops annual budgets, budget revisions and amendments as needed.
- Provide back-up and guidance for the D2C monthly data exchange activities.
- Staff programmatic meetings as needed through developing agendas, coordinating scheduling, follow-up correspondence, and taking meeting minutes.
- Serve as a liaison to medical facilities to support identifying and re-engaging patients that are disengaged in medical care.
- Provide technical support to medical facilities on re-engagement activities and during onboarding and ongoing monthly case conferences.
- Conduct/coordinate monthly case conferences meetings, draft communication and create and maintain conference schedules.
- Prepare reports and make presentation(s) to internal and external stakeholders on program activities.
- Represents AACO with other PDPH Divisions and external partners.
- Additional duties as required.

Experience:
- Three years of experience working in the HIV field with supervisory responsibilities
- Prior experience in planning, developing and coordinating a public health project
- Experience working with medical care facilities
- Experience implementing outreach activities
Education Requirement:
- MPH, MSW, MHA or master’s degree in other related field or equivalent experience
SALARY:
