What are the responsibilities and job description for the Diabetes Educator (PRN) position at Piedmont Healthcare?
JOB PURPOSE:
Functions in the specialty role of Diabetes Educator by providing direct patient care in the acute care and ambulatory settings based on consultative requests from the medical staff. Works closely with the entire medical staff and nursing staff to prepare patients for discharge in a safe, efficient manner. Provides tools and skills for homecare to patients and families. Makes recommendations to hospital staff for changes in diabetes management. Serves as an educational and informational resource to all hospital staff and the community.
MINIMUM EDUCATION REQUIRED:
Graduate of a Nursing program or Bachelors of Science in applicable field.
MINIMUM EXPERIENCE REQUIRED:
Two (2) years of experience in a clinical setting
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current License as a Registered Nurse(RN), Registered Dietitian (RD), Certified Diabetes Educator(CDE), or Diabetes Care and Education Specialist (CDCES) required
ADDITIONAL QUALIFICATIONS:
- Experience in Diabetes Education preferred
KEY RESPONSIBILITIES:
1. Provide diabetes self-management education and diabetes management recommendations to patients and families in the in-patient and out-patient settings
2. Provide education / management to patients with all types of diabetes
3. Collaborate with medical, nursing, and case management staff
4. Assess and develop plan of care, evaluate effectiveness of plan
5. Remain current on the rapidly changing field of diabetes management and the latest recommendation for patient education techniques
6. Teach all aspects on insulin management
KNOWLEDGE, SKILLS, ABILITIES:
- Content expert in all areas of diabetes management
- Skill and ability to communicate effectively both verbally and in-writing.
- Skill and ability to handle multiple priorities and deadlines.
- Ability to work as a member of a team.
- Ability to work with clients of all levels of cognitive and motor skills
- Ability to work with patients for whom English is not their first language
- Ability to problem solve and work independently
- Ability to serve as a speaker on diabetes related topics to patient community, and professional groups
- Have complete understanding of the pathophysiology and management of DM1, DM2, PreDM, PCOS, MODY and Gestational Diabetes
