As a Medical Records Technician, you will play a crucial role in ensuring the efficient organization, maintenance, and security of patient medical records. You will be responsible for processing requests for medical records from authorized individuals or healthcare providers while maintaining confidentiality of patient information.

Requirements:
- Bachelor’s degree
- One (1) year of experience in the medical records field.
- Experience processing release of information requests and legal requests
Preferred
- Three (3) years of experience in the medical records field.
Responsibilities:
- Ensure medical records are organized and complete.
- Conduct a daily audit of medical records to ensure completeness and compliance with regulatory requirements and share findings with local leadership team.
- Identify and track discrepancies or errors in medical records daily.
- Collaborate with appropriate supervisor(s) daily to resolve discrepancies or errors.
- Re-route incomplete records (including unsigned documents) to appropriate staff/supervisor and ensure record completion by working with appropriate supervisor daily.
- Monitor discharge records to ensure closure within appropriate time frames to meet standards.
- Review discharge records for completion within 24 hours of discharge (72 hours on weekends and holidays).
- Collate and assemble discharged patient records according to department guidelines within 24 hours of discharge.
- Input and maintain patient outcome data.
- Receive and log all correspondence requesting information from the medical record department daily.
- Collaborate with other departments or external entities to facilitate the release and exchange of medical record information in a timely manner, following established company and department procedures.
- Maintain compliance with State, Federal, and CARF guidelines with respect to Medical Records.
- Monitor and review release of information forms for proper completion to ensure patient rights are protected.
- Attend team meetings and complete all training courses timely as required.
