What are the responsibilities and job description for the Care Management Assistant position at Pioneers Memorial Healthcare District?
The Care Management Assistant (CMA)is Responsible for supporting the care managers (Social Workers and Nurses) in coordinating the discharge plan and securing authorizations for the stay. With direction, initiates communication with post-acute providers to identify available beds and other resources. Facilitates acquisition of appropriate insurance information and authorization prior to discharge. Secures equipment as directed. Ensures completion of all necessary post discharge paperwork and sends to provider as appropriate. Responsible for the timely delivery, documentation and tracking of Medicare "Important Message" notices to Medicare patient or representative. Assists Care Management staff with tasks necessary to meet department goals and objectives.
· High School Diploma or equivalent
· 3 years’ experience in healthcare and or insurance
· Strong communication skills
· Excellent customer service skills and phone etiquette
· Ability to function autonomously.
· Experienced in computer programs including Excel and Office.
· Ability to set and maintain priorities when dealing with multiple demands and interruptions.
· Knowledge of insurance and billing requirements
