Social Service Assistant
Position Purpose:
The purpose of this position is to implement services to meet the social and/or emotional needs that affect the residents’ ability to achieve their highest
level of function; participate in the development of residents’ comprehensive care plans; and provide social services to residents in compliance with
federal, state, and local regulations.
Essential Job Functions – Principal Accountabilities:
- Participate in family and resident activities designed to promote social interaction, reality orientation and intellectual stimulation.
- Develop one-to-one professional relationships with residents and families as needed for counseling.
- Counsel residents and families in dealing with feelings about death or dying and other emotional, mental environmental or physical limitations.
- Assist in development of individualized plans to meet residents’ needs.
- Complete the sections of the (MDS) Minimum Data Set assigned completely, timely and accurately.
- Document the social care component of the Comprehensive Care Plan for each resident.
- Document psycho-social needs, interaction, and follow-up action(s) with resident and/or family according to facility policies and procedures.
- Document residents’ discharge needs in accordance with facility discharge planning policy and regulatory requirements.
- Refer residents to social, health and community agencies and complete accurate documentation in residents’ records concerning the results of
such referrals. * Assist in coordinating family and community resources as needed to solve financial needs and to promote emotional security.
- Participate in community events related to resident’s needs and interests.
- Implement room visits for residents unable to leave their rooms.
- Cooperate with the activity department to conduct events to meet socialization needs of the residents.

Minimum Requirements Education: Education in social work or related field is desirable.
Experience: Experience performing the essential functions of the position is desirable.

Knowledge/Skills/Abilities:
- Familiarity with principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career
counseling and guidance. * Physical, sensory, and cognitive abilities sufficient to perform essential functions.
- Ability to deal professionally, courteously, and efficiently with the public and to remain calm under stress.
- Strong customer service skills.
- Good oral and written communications skills.
- Good organizational skills.
- Knowledge of all confidentiality requirements regarding medical clients and strict maintenance of proper confidentiality on all such information.
- Proficient in Microsoft Office; Ability to work independently. Must project a professional presence and appearance.

- Lift and carry up to 20 pounds frequently; up to 35 pounds occasionally.
- Stand and walk frequently; Sit for extended periods; simple grasp; fine hand manipulation; near vision; hear conversation; speak.

Pay: $16.00 - $18.00 per hour
