POWER's mission is to help women reclaim their lives from addiction and related emotional health issues and improve the well-being of future generations. We are an agency committed to providing quality gender-responsive, trauma-informed treatment and recovery supportive services to women with substance use and co-occurring disorders.
The Lead Health Navigator is a registered nurse who is responsible for the agency’s Behavior Health Home Plus Expansion (BHHPE) implementation. In partnership with Community Care Behavioral Health Organization, POWER works to help improve the overall health of its clients through wellness coaching and use of employees with lived experience, primarily Mentors and Resource Coordinators.
Essential Duties and Responsibilities
Develop a BHHPE leadership team (Quality Improvement Team).
Complete the Train-the-Trainer Wellness Coaching training; and recruit, orient, and train Wellness Coaches and trainers.
Implement Community Care’s Learning Collaborative Model.
Provide behavioral and physical health education to staff.
Identify clients in POWER’s care who are at high risk for health problems.
Ensure completion of physical health assessments for our high risk population.
Implement Self-Management strategies using web-based tools provided by CCBHO.
Use the Disease Registry to monitor progress of each participant.
Develop a process that engages and establishes collaborative relationships with PCP/medical specialty providers, and collaborate with clients’ PCPs and specialty physicians/providers to enhance client experience and overall health.
Oversee and ensure quality assurance by monitoring completion and quality of assessments, referrals, coordination of services, and action plans for clients.
Establish and maintain positive public relations with physical health providers and specialists by serving as an ambassador of the POWER organization.
Maintain compliance with the Office of Drug and Alcohol programs licensing regulations and contract requirements, including the confidentially of drug and alcohol clients.
Make recommendations to the Policy and Procedure Manual to the Clinical Director as necessary.
Together with POWER Connection Supervisors, supervise and ensure that Health Navigators meet prescribed expectations.
Assist with outcomes assessment activities and monitor program outcomes.
Participate in case conferences to support best practices
Participate in assigned meetings, staff development activities, and trainings.
Requirements
Registered Nurse with experience in community education preferred.
Valid driver’s license, use of a reliable vehicle to transport clients and documentation of required vehicle insurance.
Excellent computer skills.
Strong verbal and written communication skills.
Excellent leadership and organizational skills.
Self-directed and flexible to meet the needs of administration and management, and ability to manage multiple tasks.
Responsible for working with women in treatment in a manner that is gender-responsive and trauma-informed, to help them develop an individualized treatment plan and achieve their goals and objectives, by learning the skills necessary to live free from the burden of addiction.
Model and support clients and staff in health and wellness activities and with using healthy coping skills.
Support organization’s mission with a sensitivity of cultural diversity and workplace harmony.
Ability to secure Act 33 and 34 clearances.
Working Conditions
