Precision Spine Care is currently looking for a LVN Case Manager to join our spine and interventional pain clinic in Lufkin! The LVN Case Manager is responsible for providing clinical nurse duties while maintaining and overseeing the daily clinical policies and procedures to ensure patients benefit optimally from the clinical services received. Candidates should be comfortable in a fast pace environment, motivated, detail oriented and have excellent customer service skills.

Essential Functions & Responsibilities
- Obtains, verifies, and records health history of patients, established and new.
- Gathers pertinent health data through physical assessments and biological samples.
- Provides basic patient care as ordered.
- Assists in performing exams, diagnostic procedures and treatments, minor medical procedures and removal of lead pulls and sutures.
- Administers medications and treatments as directed.
- Documents care provided or procedures performed accurately in patient’s electronic health record.
- Documents assessments, interventions, patient/family responses, medication dispensed/prescribed, and test results in patient’s electronic health record.
- Enters orders from the physician/provider for Urine Drug Screens; ensuring results are routed for review.
- Collects specimens for laboratory analysis.
- Ensures routing slips are complete in a timely manner.
- Retrieves radiology results and routes them appropriately.
- Completes the Triplicate prescription form for physician review, approval and signature.
- Participates in collaboration with others on plan of care development and safely implements that plan.
- Promotes wellness by providing patient education materials, communicating physician advice/instructions.
- Receives phone calls and contacts patients addressing concerns, issues and postoperative questions.
- Oversees the physician schedule- responsible for marking time off and changes.
- Maintains adequate daily flow of team; keeping self and other clinical support staff accountable.
- Work constructively with Clinic Manager and Director of Operations.
Qualifications
- Current, unencumbered nursing license in the state of Texas.
- Current CPR certification.
- Prior knowledge and use of NextGen electronic health record system a plus.
- Be able to read, write, and speak fluent English.
- Strong PC skills and working knowledge of Microsoft Office.
- Effective verbal and written communication skills and the ability to present information clearly and professionally.
- Strong interpersonal skills.
- Skills to manage stress in order to maintain a professional atmosphere and provide quality service.
- Regular, dependable, and reliable attendance.
