What are the responsibilities and job description for the CCU North Care Coordinator (Choice Program) position at Premier Home Health Care Services, Inc.?
Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have an opportunity for a Care Coordinator in Chicago, IL for our Choice Program. We are looking for a dynamic individual to conduct determination of eligibility/need assessments, facility placement pre-screening assessments, and authorize Community Coordination Program (CCP) Services for all eligible Illinois Department on Aging’s CCP program participants.
Key Functions include:
- Administration of the appropriate intake form, including a comprehensive needs assessment
- Development of a case plan
- Making appropriate referrals and responding to applicant/client requests
- Authorization of services
- Maintaining case records, including documentation of follow-up, reassessment, and termination.
Qualifications:
- Must possess a BA/BS degree in social science, social work or related field.
- Two years of program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly.
