Position Summary:
The Hospice Case Manager, RN is responsible for providing skilled nursing care services to patients through Hospice serving a delineated geographic area under this specialized program. This includes providing nursing care to patients in their residence on the basis of the individualized Hospice Plan of Care developed and monitored by the Hospice Interdisciplinary Team that includes the primary physician. The Hospice Case Manager requires substantial specialized knowledge, judgment, and skill based upon the principles of biological, physiological, behavioral and sociological sciences. The Hospice Case Manager uses critical thinking skills in making decisions regarding patient care that includes knowledge and integration of relevant standards, resources, and data. The Hospice Case Manager delegates the performance of selected patient care tasks to competent individuals in selected situations. The Hospice Case Manager delegating the task retains the responsibility and accountability for the care and coordination of care for the patient.
Requirements:
- At least two years of work experience as a professional nurse preferably at least one of the years in hospice care
- Ability to assess and respond to the needs of patients and loved ones/caregivers in varied settings.
- Skilled in nursing practice, able to cope with loved ones/caregivers' emotional stress and tolerant of individual lifestyles
- Ability to make meaningful observations, skilled assessments, and write comprehensive, accurate reports
- Must be a team player who supports an interdisciplinary team concept of caring for patient and loved ones/caregivers
- Must have a valid driver's license, a good driving record, the use of a reliable vehicle, and the ability to drive year-round to locations
- Must be able to take direction, work independently and effectively as part of a multidisciplinary team in a positive, professional manner which supports the mission of the organization
