Collects pertinent assessment data that includes present condition, past history, physical, spiritual and psychosocial information utilizing input from patient, significant other, and/or healthcare tem members as applicable. Interprets data to identify the patient’s needs and potential problems.
Develops, reviews, and revises an individualized care plan within the time frame identified in documentation standards. Coordinates care with medical oncology and radiation oncology, and other disciplines (i.e. inpatient, outpatient and assisted living areas). Establishes expected outcomes that are measurable and achievable.
Evaluates patient’s response to care and initiates independent and/or collaborative changes where appropriate. Triages as needed for any immediate interventions necessary and implements appropriate actions based upon assessment. Documents changes in condition and response to interventions.
Utilizes nursing process to incorporate teaching into delivery of patient care. Documents teaching and learning.
Maintains current knowledge of nursing practice by attending continuing education, serving on hospital committees and/or completing institutional and unit-specific competencies. Maintains annual clinical and institutional competencies.
Utilizes nursing process to incorporate teaching into delivery of patient care. Documents teaching and learning. Delivers patient care based on pathophysiology and nursing principles. Performs technical skills with accuracy and efficiency. Establishes and adjusts priorities based on patient acuity and changing clinical situations.
Recognizes the patient’s age as a relevant factor and applies appropriate decision-making skills in the assessment, treatment, and care provided to the patient.
Communicates effectively with patients, families, physicians and coworkers. Is a patient advocate. Utilizes resources effectively.
Documents complete and accurate information on all appropriate chart forms and computer applications in a timely manner. Assures appropriate billing of services provided.
Delegates, effectively utilizing the skill level of the department members, respecting and supporting their input.
Provides nursing care to patients receiving assessment, treatment and supportive care for cancer utilizing the nursing process: assessment, planning, implementation, and evaluation.
Provides nursing care to outpatients requiring hydration, medication infusions, chemotherapy, blood products, and procedures utilizing the nursing process: assessment, implementation, and evaluation.
Communicates effectively with patient, families, providers, clinical team members, and coworkers. Is a patient advocate.
Participates in department quality improvement activities.
Documents complete and accurate information on all appropriate chart forms. Documents phone interventions/recommendations when pertinent to patient’ ongoing plan of care.
Demonstrates support for the Swedish Cancer Institute mission, vision, values, and goals.
Provides office support. Assumes responsibility for management of the surgical oncologist’s clinic as established by each network patient care team. Assists provider(s) with exams and biopsies.
Performs nursing duties within accepted standards of practice, including the Washington State RN Practice Act and established hospital policies and standards.
Demonstrates a positive attitude; able to work with all levels of employees in a respectful and supportive way. Must be able to work collaboratively with colleagues, anticipating needs, and offering support. Exhibits flexibility as needed to effectively and efficiently cover clinical functions on a daily basis.
Demonstrates emotional stability with the ability to adapt to rapidly changing and stressful situations, and fosters a calm, professional environment for patient care.
Participates in department quality improvement activities.
Required Qualifications:
Graduation from an accredited nursing program.
Upon hire: Washington Registered Nurse License
Upon hire: National Provider BLS - American Heart Association
Within 90 days of hire: Successful completion of the Fundamentals of Chemotherapy Immunotherapy Administration
1 year Nursing experience.
Preferred Qualifications:
Upon hire: Oncology Certified Nurse (OCN)
