Raze Therapeutics is seeking a qualified and motivated Physical Therapy Assistant for a 13-week contract assignment in Lakeport, CA. The ideal candidate will have an active State License and a passion for helping patients reach their rehabilitation goals.

Responsibilities:
Assist Physical Therapists in providing physical therapy services to patients in accordance with established plans and goals
Collaborate with interdisciplinary teams to ensure optimal patient care and outcomes
Maintain accurate and up-to-date patient records and progress reports
Comply with all applicable laws, regulations, and standards related to physical therapy
Requirements:
Active State License as a Physical Therapy Assistant
Strong clinical and communication skills
Ability to work effectively as part of a team
Ability to maintain accurate and detailed patient records
Passion for helping patients reach their rehabilitation goals and improve their overall quality of life
Completed at least one 13-week assignment in a skilled nursing facility (preferred, not required)
Must be vaccinated for Covid-19, have proof of a medical exemption, or complete a religious exemption form upon orientation
Must wear an N-95 mask while inside the facility if not fully vaccinated
