Primary Responsibilities
The Social Worker will be responsible to work with the residents in overseeing and assisting in the admission process, identification of needs and problems, make referrals to community resources, and ensuring the mental and social health of each resident.
Essential Functions
- Develop relationships with residents and their responsible party as needed for admissions, counseling, and identifying needs
- Work with responsible party and resident on social interaction, reality orientation, and intellectual stimulation
- Assist Business Office Manager in solving financial needs and promote emotional security for the resident
- Complete acute care plans as needed and attend care plan meetings
- Assist Administrator with resident/family/responsible party concern management
- Attend committee meetings as required.
- Complete monthly QA reports.
- As needed, refer residents for vision, dental, and hearing services
- Assist with room changes by contacting responsible party and resident and completing necessary forms
- Complete the initial, routine, and as needed psychosocial and socialization assessment, if needed the LSW can refer resident for additional professional psychosocial services.
- Discuss advance care planning with responsible party upon admission and as per state regulations and complete necessary documents
- Make morning rounds prior to morning meeting
- Be prepared to provide staff with any important department updates at the department head daily morning meeting Perform suicidal/homicidal assessments and required follow up Assist in investigations as requested by Administrator
- Assist with the completion of the MDS as needed
- Work rotating weekend Manager on duty as scheduled
Qualifications
Educational/Training Requirements

Bachelor’s or Master’s in Social Work
Licensing Requirements
-
Must be a Licensed Social Worker in the state of Texas
Experience Requirements
-
Previous experience working in a skilled nursing facility is preferred
