Job Title: Principal Care Management Coordinator
Job Summary:
The Principal Care Management Coordinator is responsible for overseeing and managing the care coordination process for patients within the Rio Grande Urology system. This role involves developing, implementing, and monitoring individualized care plans, ensuring compliance with healthcare standards, and coordinating with various stakeholders to optimize patient outcomes.
Key Responsibilities:
- Care Coordination: Develop, implement, and monitor individualized care plans for patients, ensuring that all aspects of care are coordinated efficiently and effectively.
- Patient Advocacy: Serve as the primary point of contact for patients and their families, advocating for their needs and ensuring they receive appropriate care.
- Collaboration: Work closely with healthcare providers, including doctors, physician extenders, and other care team members, to ensure a holistic approach to patient care.
- Compliance and Documentation: Ensure that all care coordination activities comply with RGU established protocols and standards. Maintain accurate and up-to-date patient records, including care plans, progress notes, and communication logs.
- Education and Support: Provide education and support to patients and their families regarding their care plans, treatment options, and available resources.
- Communication: Facilitate communication between patients, healthcare providers, insurance companies, and other relevant parties to ensure seamless care delivery.
Qualifications:
- Education: Licensed Certified Medical Assistant is preferred.
- Experience: Minimum of 2-3 years of experience in healthcare setting.
- Licensure/Certification: Active CMA license, LCSW, or other relevant certification is required.
Skills:
- Strong knowledge of care coordination and case management principles.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health records (EHR) and care management software.
- Problem-solving and critical thinking abilities.
Reporting:
- Reports to the RGU Executive Director.

Pay: From $17.00 per hour
