Overview
The Nurse Practitioner role at RiverSpring Star - Institutional Special Needs Plan (ISNP) of RiverSpring Health Plans, will be responsible for providing primary medical care and care management/coordinator to a panel of assigned RiverSpring Star Members residing in our contracted Skilled Nursing Facility the Hebrew Home at Riverdale located in Riverdale - North Bronx.
Responsibilities
Position is responsible for providing primary medical care and care management/coordination to a panel of assigned RiverSpring Star members residing in the contracted Skilled Nursing Facility. S/he will collaborate with the member's PCP and the nursing home Medical Director. S/he will conduct regular and acute visits / assessments in accordance with the Medical Staff by-laws of the Skilled Nurse Facility (SNF) and current Federal and State Health Code. S/he will work with the nursing home ICT (Interdisciplinary Care Team) members.
Qualifications
Ideal candidate must possess a graduate degree of a Nurse Practitioner program. Must have a minimum of two years full time experience as a Nurse Practitioner, preferably in clinical geriatric care. Ability to work independently, develop a differential diagnosis and initiate treatment. Board Certification from AANP or ANCC. Current DEA Certificate, BCLS.
Compensation : Min. $120,000 - Max. $140,000
