ASUMMARY

Conducts face to face assessment of participants in the community, facilities, or PACE center. 

STATUS:  Full Time             

LOCATION: ElderONE       

    

ATTRIBUTES

  • Current New York State Driver’s license and availability of motor vehicle
  • Excellent ability to follow direction and be flexible to change
  • Exceptional communication and interpersonal skills
  • Compassionate, warm and patient focused
  • AAS Nursing Degree Required, BA in science of Nursing preferred.
  • 6 months- 1 year experience conducting UAS-NY Community Assessment preferred.

RESPONSIBILITIES

  • Collaboration. Works collaboratively with the IDT to implement interventions based on outcomes of UAS. Participates in the development and revision of the patient’s plan of care as a member of the multidisciplinary team. Ensures home care plan and service are provided as ordered and that they are consistent with established participant goals.
  • Participant Care. Regularly reviews the participant plan of care to ensure adherence to the interventions. Shares outcome of each UAS with IDT to develop plan of care. Responsible for coordinating the delivery of 24-hour care.
  • Evaluation & Documentation. Conducts reassessments and evaluations of each patient on a regular basis and at significant change in condition and provides skilled nursing care for ongoing or episodic illnesses, according to the plan developed by the team. Maintains complete and accurate clinical documentation within established agency/program time frames.

Qualifications:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
