What are the responsibilities and job description for the Home Health Aide position at RPRC Services LLC?
We are seeking a 1099 Home Health Aide (Per Diem) to join our team! You will be responsible for the assessment, evaluation, and treatment of assigned patients in a home health setup. Let us know which areas you can service and we'll match you with the right referrals!
Please note that our rates are per visit/case basis.
Responsibilities:
- Providing nurse visits to home health patients. (home visits)
- Administer nursing care to ill, injured, or disabled patients
- Evaluate and establish patient treatment plans
- Monitor and report changes in patient symptoms or behavior.
- Communicate with collaborating physicians or specialists regarding patient care
- Educate patients about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members
Qualifications:
- Previous experience in nursing or home health
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
Requirements:
- Professional Home Health Aide License (We cannot accept neither Certfied Nurse Aide (CNA) nor Home Care Aide (HCA) for this position)
- Academic Diploma
- Updated CV/Resume with Experience
- Driver's License
- Auto Insurance
- Active BLS or CPR Certification
- Recent Physical Examination OR Fit-to-Work Note from Physician
- Valid (1 year) TB/PPD Test OR Valid (3 years) Chest Xray
- Covid Vaccine Card/Records (Includes Booster/s)
- Influenza Vaccine (or Declination)
- MMR Immunization Records
- Hepatitis B Immunization Records
- Photo (For Issuing of L&G Nursing Group ID)
- Reference Check (at least 2 credible referees)
