Rutgers, The State University of New Jersey is seeking a Patient Navigator for the Department of Medicine-Infectious Disease within the New Jersey Medical School. The primary purpose of the Patient Navigator position is to provide clinic and community-based HIV, STI testing, and case management services to persons with and at risk for HIV infection. Performs a biopsychosocial assessment and develops an informed care plan to address any identified social service needs and treatment adherence barriers. As part of the team, the Patient Navigator will provide services in clinical settings and on the mobile health unit including working select weekend and evening hours. Maintains the confidential integrity of all client level information. Proactively reports any issues requiring action, intervention, or resolution to the Program Manager in a timely manner.
Among the key duties of this position are the following:
- Provides a confidential assessment for those clients living with HIV and clients being referred for PrEP services determine the HIV prevention needs of the clients whether their test results are positive or negative; this includes the registration and planning of prevention services.
- Provides HIV and STI confidential pre and post-test counseling for identified patients and others according to NJ Department of Health and Senior Services and Centers for Disease Control and Prevention guidelines and documents accordingly.
- Coordinates PrEP and PEP for clients.
- Provides medical case management for newly diagnosed persons with HIV infection.
- Maintains documentation of all client encounters and complete reporting requirements according to grant and department standards.
