Qualification/Responsibilities:

• Must possess an unencumbered license to practice as a Registered Nurse
• Must possess, as a minimum, a Nursing Degree from an accredited college or university
• Must have Precision in care Expertise in Assessment 
• Conduct and Coordinate the development and completion and the resident assessment (MDS) in accordance with current regulations and guidelines
• Must be experienced in PDPM, MDS coding, insurance updates, care planning 
• Long-Term Care Experience Required
• Assist in completion of patient discharge
• Auditing the resident assessment (MDS) and care plan for completeness and accuracy.
• Develop, implement and maintain a quality assurance program resident care plan (Case Management and Care Planning)

Shift & Wage:

• Full-Time Opportunity Available
• Shift Needed: Monday-Friday, 8:00am-4:30pm
