About the team:
The Home Care team at Sage Eldercare is dedicated to delivering exceptional in-home care and fostering a supportive environment for our Personal Care Assistants (PCAs) and clients. We meticulously screen, train, and onboard our PCAs, ensuring they are thoroughly prepared to provide the highest level of care.
By developing clear, individualized care plans, we oversee the implementation of tailored care and meaningful Quality of Life activities. Our team builds respectful relationships with both PCAs and clients, offering constructive feedback and inspiration to encourage active engagement.
Working in close collaboration with our Care Managers and the broader Sage team, we guarantee seamless, comprehensive care for our clients. Driven by a commitment to excellence, we are dedicated to achieving the best possible outcomes for those we serve – both our clients and our valued employees.
What you’ll do:
- Screen, train, and onboard PCAs.
- Develop individualized written care instructions.
- Provide oversight and direct supervision of PCAs’ work with clients.
- Ensure PCAs’ successful implementation of QOL activities.
- Effective collaboration with the comprehensive Sage team to ensure seamless coordination of care.

- You provide specific, timely, and constructive feedback and guidance to help PCAs improve their performance.
- You creatively inspire and motivate PCAs to engage actively in Quality of Life activities.
- You excel at translating known information into clear and detailed written instructions.
- You collaborate with Care Managers, PCAs, and Activity Specialists to develop creative strategies to overcome barriers to care, capturing these strategies in comprehensive care instructions.
- You create a positive experience for both candidates and employees, leaving candidates excited about the prospect of joining Sage and employees enthusiastic about referring colleagues to apply.

This position has a target start date of October, 2024.
Compensation: The anticipated base compensation for this position is between $71,302 - $83,200 DOE annually. Offered compensation will be determined based on education and experience.
Salary : $71,302 - $83,199
