The Job

The VP National Integrated Primary Care will provide clinical leadership and oversight of team members on the SCAN Mobile Care Team and provide direct primary care to empaneled SCAN members.


You Will

Provide clinical leadership and oversight of Scan Mobile Care Team members to achieve desired person-centered outcomes for members.

Work collaboratively with other department leaders including case management, operations, and NP leadership to design and execute care models to enhance value-based care delivery capabilities for SCAN members.

Participate in strategic planning for growth and scale of Integrated Primary care at scan including but not limited to expansion into new geographies, care delivery to members of other products, and support for medical groups participating in value-based relationships with SCAN.

Provide leadership and oversight of Regional Integrated Care leads and hold accountable to performance metrics.

Provide care in collaboration with a care team to an assigned panel of members in their place of residence.

Maintain a panel size of 250 members assigned primary care member

Collaborate with Integrated Care Nurse Practitioners and other team members to manage complex patients by regularly seeing members in tandem and being available for urgent telephonic or virtual engagement.

Serve as a liaison with other primary care physicians, hospitalists, ED physicians and specialists to coordinate care for post-acute care patients.

Prescribe medication in accordance with California statute and professional practice guidelines.

Participate with Integrated Care Sales team in events to build member awareness and educate communities.

Participate in on-call program to support team members telephonically after hours and weekends.

Participate in peer review, quality assurance, provider meetings, and other clinical meetings.

Work closely with the SCAN Mobile Care Team management as it pertains to clinic operations and responsibilities.

Your Qualifications

  • Medical Doctorate Degree required. Current active Doctoral License in good standing required.
  • Geriatrics/Palliative care experience preferred
  • 4 years of related experience as a physician, preferably with home-based visits, geriatrics, chronic disease management, and hospice and/or palliative care.
  • 2 years of primary care leadership experience preferentially in a value-based care delivery system.
  • Experience with virtual care/telehealth preferred.
  • Knowledge of the treatment of complex chronic diseases, such as dementia, diabetes, heart failure, and COPD.
  • Ability to identify team needs and coordinate with other resources/programs appropriately.
  • Knowledge of HCC and ICD-10 coding and documentation.
  • Knowledge of wound care and management.
  • Knowledge of CMS guidelines and Medicare Advantage Managed Care.
  • Comfortable with initiating goals of care and end of life discussions.
  • Strong interpersonal skills and change agent mindset needed to build relationships with healthcare team and patients.
  • Cultural competence and sensitivity required, multilingual skills preferred.
  • Knowledge of and ability to maintain HIPAA requirements.
  • Adaptable and open to change and problem-solving.
  • Knowledge of and/or ability to learn basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and open to new technology
  • Moderate Driving as needed
  • Work mode- Mostly remote
