What are the responsibilities and job description for the LVN/LPN position at Sentido Health?
The LVN Field Nurse is responsible for working in collaboration with the Director of Nursing and the Assistant Director of Nursing, to provide Nursing care, support, and intervention to medically fragile patients/families in their homes. The LVN Field Nurse is responsible for following the Plan of Care for the individual patient, along with all Physician orders. The LVN Field Nurse must be able to work independently and in collaboration with the team, patient and family to accomplish goals and objectives as outlined in the Plan of Care.

Required skills
- Licensed by the Board of Nursing as a Licensed Vocational Nurse.
- Demonstrate knowledge and skills necessary to provide care and communicate with the pediatric population and families served by the agency.
- Ability to assess data reflecting the patient’s status and interpret it appropriately to provide specialized care for the patient.
- Familiarity with home care rules and regulations by the Agency.
- Must have and maintain a current CPR Certificate.
- Must be competent in computer applications and have the ability to readily learn the Agency’s specific software.
Competencies
- Accurate and timely documentation in Kantime and other systems.
- Effectively communicates to patients, families, Case Managers, DON, ADON and Physicians.
- Maintains clean vehicle and up to date insurance and registration.
- Maintains presentable appearance, properly follows PPE Guidelines.
- Medication Administration via the following routes: PO, GB, GJ, Rectal, IM, IV, SQ, Nasal, Optic, Otic, and inhaler.
- Wound Care
- GB/GJ Care/Maintenance, Delivery of medications and feeding via GB/GJ, Feeding pump use.
- Tracheostomy care/maintenance, Suctioning: Oral, Nasal, and Tracheostomy, Ventilator use, HHN administration, Oxygen Administration, Pulse Oximeter Use.
- Urinary Catheterization insertion and removal.
- Universal Precautions, Safety Measures, Seizure Precautions.

Pay: $18.00 - $22.00 per hour
