Reports To: Clinical Director or Director of Patient Care Services
Job Description Summary
Responsible for coordinating all aspects of the patient intake process including maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes. Collaborates with the Director of Patient Care Services and Account Executives/VP Busines Development to ensure timely and appropriate admissions.
Essential Job Functions/Responsibilities
- Coordinates daily patient referral and intake operations with DPCS and Account Executives.
- Enters patient information into EMR, including detailed referral information as it relates to patient transition to home care.
- Maintains patient confidentiality and protects operations by monitoring confidential information.
- Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.

Maintains positive working relationships with current and potential
referral sources. - Ensures seamless transition of patients to home care by providing patient education and preparation for home care and coordination of care with multiple service providers.
- Participates in insurance verification and authorization processes.
- Assists in assessing DME needs and pacing orders for approved DME.
- Gathers, collates, and reports referral statistics including key customer referral trends.
- Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by the organization.

Position Qualifications

At least two (2) years experience in health care management, preferably in home
care operations. - Demonstrated ability to maintain performance under a fast-paced, heavy work environment.

- Demonstrates good communications, negotiation, and public relations skills.
- Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Salary : $41,100 - $52,100
