Sevita is launching a Whole Health Care model (physical behavioral cultural social spiritual) to meet the needs of the individuals served in on-site clinics, virtual and mobile methodologies integrated with social, community, rehabilitative services and long term supports. The Whole Health Care model paving the way for the future of health care for individuals with intellectual and developmental disabilities (I/DD).
The Registered Nurse RN Care Coach’s primary role is developing and implementing complex care, transitional care and condition care coaching and management for individuals served that incorporate whole person care including medical, behavioral, cultural, social and spiritual care in collaboration with an interdisciplinary team.
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:
- Perform individual served care management and coaching including but not limited to complex care, care transitions and care coordination activities.
- Develop a plan of care based on whole person needs incorporating medical, behavioral, social and spiritual needs in collaboration with a whole health care team that includes nursing and medical interventions (as appropriate) and measurable goals and/or outcomes leveraging evidence-based theoretical approaches.
- Routinely review pertinent individual served medical, behavioral and other pertinent data to determine the effectiveness of care and achieving healthiest life in the most natural setting.
- Educates individuals served, family and caregivers to empower to the individual served to live healthiest life.
- Coordination and advocacy of individual served care through the timely interventions following whole health model processes.
- Follow organizational policies, procedures, and practices pertaining to professional nursing care and care coaching.
- Participate in whole health care meetings and other designated activities as required.
- Perform other duties and actions as required and appropriate to support whole person care.

Education and Experience:
- Associate’s or Bachelor’s Degree in Nursing
Certificates, Licenses, and Registrations:
- Valid nursing license in State of Texas
