Looking for a outstanding Licensed Practical Nurse (LPN) to visit several home care patients. We provide our patients with kindness and healthcare support in their home setting. This is a great opportunity to use your nursing skills to develop a relationship with your patients and offer support to their families.

Our ideal candidate will have excellent nursing judgement, a positive attitude, compassion and impeccable bedside manner. This is a fast passed environment and requires knowledge of multiple disease processes, treatment plans, medication and education.

* Flexibility in scheduling, we offer our nurses the opportunity to choose between seeing the same patients twice a day (once in the am, once in the PM), three day a week visit schedule and daily visits.

* Education and support. Our field nurses have access to supervising nurses for support. And yearly continuing education.

Responsibilities

• Participate in creating and following the patients plan of care with support from the RN and physician.
• Accurately report/document the patient's condition and care provided.
• Prepare clinical/progress notes and summaries, in a timely manner.
• Seek guidance from the staff RN when needed.

Qualifications

• 0-1 year experience as an LPN in a home care setting
• Current state license as an LPN
• Graduate of an accredited program of practical nursing
• Proof of current CPR certification
• Excellent interpersonal skills
• Valid driver’s license and reliable transportation
• Ability to work a flexible schedule and travel locally. All patients are in the same area.
