SOCIAL SERVICES DIRECTOR
Job Description:
Planning, organizing, implementing, evaluating and directing the overall operation of the Social Service Department. The Social Services Designee will provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psycho-social well-being.
Job Responsibilities:
· Resident/Family/Physician Intervention
· Psycho-social Services
· Vision, Hearing, Dental, Podiatry Service Referrals
· Assessments & Care Planning
· Discharge Planning

Requirements:
