What are the responsibilities and job description for the Program Director: Case Management Services position at SOUTH COMMUNITY INC?
The Program Director champions Care Coordination as our model for case management and is responsible for clinical and administrative oversight of leadership and teams providing Youth and Adult Case Management Services including Supported Employment. This is a full-time position reporting to the Chief Operations Officer.
Principal Duties and Responsibilities:
- Plans and directs clinically and administratively all aspects of Adult and Youth Case Management Services, including clinical, administrative and financial. Champions Care Coordination as South Community's model for case management. Provides supervision and leadership to two (2) Co- Managers for Adult Community Psychiatric Support Services, one (1) Program Manager for Youth Community Psychiatric Support Services and two (2) CIT Co-Responders.
- Represents SCI at external collaboration meetings in the community as designated for this position.
- Participates in and leads interal collaboration at SCI, and participates in Program Council and Leadership Council.
- Assesses, plans and implements cost effective measures in dirercting programs.
- Ensures agency initiatives, strategic goals and quality indicators for the agency are implemented and sustained within Case Management Services, inclding productivity.
- Monitors and ensures fidelity of IPS as the evidence-based interventions Supported Employment Services.
- Ensures that all services are consistent with standards in the field and clinical research, ethical and procedural standards, and best practice guidelines and are within staff's scope of practice.
- Demonstrates an understanding of Local, State and Federal policy and industry changes that impact the agency, and is proactive in responding to such changes.
- Presents South Community as a professional accessible organizaton that promotes harmonious relationships with client's staff and external customers.
- All other duties as assigned.
Minimum Qualifications:
- Must have a Master’s degree or higher and must be licensed in the State of Ohio as an independent counselor or social worker (LPCC/LISW, supervisory endorsement preferred) or as a Psychologist.
- Experience in mental health and substance use treatment, supervision and demonstrated leadership are required.
- Access to an operational vehicle, full coverage automobile insurance with $100,000/$300,000 liability coverage and a valid driver's license are required. Must be able to pass a background check, motor vehicle check and drug test. Must maintain a current NPI number if applicable to your position.
