EDUCATIONAL REQUIREMENTS CHANGED - GERIATRIC SUPPORT SERVICES COORDINATOR/ INTAKE SPECIALIST

FULL TIME / 35 HOURS PER WEEK

Duties would include but not limited to the following: Complete initial in-home assessment of health and functional status for elders enrolled in a Senior Care Organization (SCO). Conduct initial in-home assessments to SCO’s enrollee(s). Responsible for maintaining caseload. Complete the necessary assessment paperwork and follow-up documentation in SAMS. Prepare member files to pass onto the ongoing GSSC. Develop and implement an Initial Individualized Plan of Care (ICP) in agreement with Primary Care Team (PCT). Contact provider(s) and arrange for in-home social support services based on ICP. Attend meetings with the (PCT) on regular basis to discuss the enrollee(s) status and care plan. Provide assistance to SCO’s RN with transitioning the enrollee(s) to/ from nursing facility, as needed. Contact provider(s) monthly by telephone to confirm ICP and initiate changes as needed. Communicate with primary care physician (PCP) and Primary Care Team (PCT) on regular basis. Provide access to public benefits and community services as needed. Develop and maintain a professional relationship with SCO’s staff, community agencies, hospitals, and nursing facilities. Meet with SCO Team Leader on regular basis. Maintain a working knowledge of ASAP and SCO’s regulations.

Qualifications: Bachelor’s Degree preferred, but not required. Bachelor’s Degree to be obtained within three years of the start date. Two years professional experience in the care of persons over age 65, with one of the years involving work in a setting where persons over age 65 receive health care services (such as a hospital, nursing facility, or other community health agency which might include health-related programs within an ASAP). Home Care Case Management experience; and advanced computer skills. Asian language capacity preferred. Must have a valid driver’s license and provide own transportation.

SALARY: $51,202
