As part of our exceptional team, you will report directly to the Executive Director. Responsibilities include:
- Serve as point person to ensure referrals and intake processes are completed efficiently and timely in coordination with area facilities.
- Provide education regarding our hospice services to area providers.
- Complete patient assessments and admissions processes as appropriate.
- Maintain thorough clinical record documentation.
- Participate in company quality assessment and performance improvement programs.
