JOB SUMMARY:
A professional who assists the Agency with managing new patient admissions through maintaining viable partnerships with referral sources and discharge planner.
QUALIFICATIONS:

RESPONSIBILITIES:
- Understands and adheres to established Agency policies and procedures and functions only within scope of practice outlined in job description
- Receives inbound intake calls from community partners and only declines new patient referral with permission of Director of Nursing or Program Director
- Responsible for accepting new patients based on insurance eligibility, staff availability and patient’s geographic location
- Utilizes Epic system to accept referrals, obtain admission and resumption of care documents
- Assigns all new admissions to appropriate clinical discipline utilizing admission checklist.
- Sends all new admissions and resumptions to scheduling coordinator via email
- Assigns evals per frequency messages
- Completes patient reassignments for case managing entities as needed based on staffing availability
- Daily monitoring of pending patients. Ensure pending patient documents are sent to Scheduling Coordinator via email
- Assigns all clinical staff in accordance with timeline outlined in agency policy
- Completes admission report daily to ensure patient assignments have been completed
- Completes transfer report MWF in effort to monitor hospitalization status of transferred patients
- Documents contact regarding patient in communication note in patient’s chart
- Fosters collaborative partnership with marketing team
- Investigates clinical assignment accuracy
- Assists in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, accrediting bodies, state and local laws
- Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performance
- Observes confidentiality and safeguards for all patient related information.
- Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regard to time involved
- Serves as a resource person to referral sources, discharge planners and clinical staff
- Maintains viable and dynamic relationships with referral sources and discharge planners
- Immediately reports any accident, incident or unusual occurrence to Program Director/ Director of Nursing
- Assists in the development of new intake process and contributes role specific insights regarding improvement of current processes
- Other duties as assigned by Program Director/ Director of Nursing
- Understands and promotes principles of continuous performance improvement
WORKING ENVIRONMENT:
Works indoors in the home health office.
