Job Responsibilities:
- Coordinates and completes MDSs according to facility policy, state and federal regulations.
- Develops and maintains the MDS schedules.
- Develops, updates, and maintains care plans according to the state and federal regulations.
- Works with the interdisciplinary team to determine appropriate timeframes for assessments and completion of assessments on Medicare A residents.
- Completes all assessments related to care planning.
- Transmits completed assessments to the state in the appropriate timeframe.
- Participates in care plan team/family meetings.
- Notifies the LNHA of any issues related to the MDS, sentinel events and QI reports.
- Adheres to Standards of Attendance.
- Performs other duties, as assigned.
Job Requirements:
Experience:
- Minimum of five (5) years experience in medical/surgical, rehab, or geriatric nursing including two (2) years as a charge nurse in geriatric care.
- Knowledge of federal and state regulations for long-term care desired.
- Previous MDS experience required.
Education:
- Current Registered Nurse License from the State of Pennsylvania
- Current CPR and IV certification.
- Bachelor of Science in Nursing preferred.
