Job Responsibilities:

  • Coordinates and completes MDSs according to facility policy, state and federal regulations.
  • Develops and maintains the MDS schedules.
  • Develops, updates, and maintains care plans according to the state and federal regulations.
  • Works with the interdisciplinary team to determine appropriate timeframes for assessments and completion of assessments on Medicare A residents.
  • Completes all assessments related to care planning.
  • Transmits completed assessments to the state in the appropriate timeframe.
  • Participates in care plan team/family meetings.
  • Notifies the LNHA of any issues related to the MDS, sentinel events and QI reports.
  • Adheres to Standards of Attendance.
  • Performs other duties, as assigned.

Job Requirements:

Experience:

  • Minimum of five (5) years experience in medical/surgical, rehab, or geriatric nursing including two (2) years as a charge nurse in geriatric care.
  • Knowledge of federal and state regulations for long-term care desired.
  • Previous MDS experience required.

Education:

  • Current Registered Nurse License from the State of Pennsylvania
  • Current CPR and IV certification.
  • Bachelor of Science in Nursing preferred.
