Member Advisor, Pathways to Progress - St. Francis Community Services

While following the mission, vision and core values of St. Francis Community Services, the Pathways to Progress Member Advisor is responsible for providing long-term case management services that help clients of the program (“Members”) leave poverty behind as a way of life. The Member Advisor conducts comprehensive intakes and assessments of Members’ needs; develops, implements and monitors individualized service plans; connects Members to appropriate community support services; evaluates the progress of Members and their children; and documents services and their impact in Members’ case files.

DUTIES & RESPONSIBILITIES

  • Understand the service delivery models associated with the Pathways to Progress program, namely Strengths-Based Case Management and Stages of Change.
  • Receive referrals and conduct screening interviews with prospective Members to determine eligibility for the program.
  • Maintain an assigned caseload of not more than 15 Members and their households.
  • Perform comprehensive intakes and assessments of newly enrolled Members.
  • Create and monitor individualized service plans for and with Members that address individual needs, updating these plans every 90 days.
  • Make visits to Member homes at the frequency indicated in each service plan.
  • Maintain contact with Members via text, calls or email, being proactive in reaching out to those who struggle to meet their commitments to home visits or meetings.
  • Serve as a coach/mentor to help facilitate positive change in Members’ lives by helping them build life skills, gain knowledge, access resources, engage in workforce training and strengthen their financial well-being.
  • Collaborate with other internal and external agency case managers to link Members with needed support services.
  • Collaborate with the SFCS attorney assigned to Pathways to address Members’ legal issues, which may include accompanying them to court appearances and ensuring that required community service is performed.
  • Assist with the coordination and delivery of financial education and workforce development sessions, possibly including the transportation of Members in company vehicles.
  • Respond to crises in Members’ lives within program parameters.
  • Monitor Members’ progress by administering the Arizona Self-Sufficiency Matrix,Readiness Ruler and Pediatric Symptom Checklist at prescribed intervals.
  • Actively participate in staff meetings to discuss cases, keep abreast of program developments, train on evaluation models, and learn about new resources
  • Participate in Pathways to Progress Advisory Council meetings as requested.
  • Assist with outreach efforts to build the Pathways to Program brand and caseload.
  • Maintain current documentation and case records as required by program, agency, and grant standards.
  • Input Member data into Salesforce database, keeping electronic records current.
  • Participate in staff training and development opportunities.
  • Other duties as assigned.

KNOWLEDGE & EXPERIENCE REQUIREMENTS

1. Demonstrated commitment to working with low-income individuals/families struggling with social and economic challenges.

2. Bachelor’s degree in social work, human services, psychology or related field; master’s degree preferred.

3. At least two years of case management experience, working with impoverished people with complex needs.

4. Bi-lingual in Spanish and English - preferred
5. Demonstrated comfort working with an ethnically and socially diverse client base in the field (specifically, making home visits).
6. Valid State driver’s license and current automobile insurance.
7. Current chauffeur’s license.
8. Acceptable background check, as performed through the Missouri Family Care Safety

Registry (this screening will be done annually thereafter by agency while employed).

RELATIONSHIPS REQUIREMENTS

1. Frequent contact with the Pathways to Progress Program Director.
2. Daily contact with program Members, requiring the ability to initiate and maintain a respectful, trusting relationship with them.
3. Daily contact with Pathways staff (and perhaps, volunteers), requiring the ability to be part of a team, coach/mentor where appropriate, solve problems, offer support/encouragement, etc.

4. Frequent contact with community partners and other community representatives.

RESOURCES FOR WHICH ACCOUNTABLE

1. Accountable for use of a purchase card, which includes submitting receipts and proper coding for each transaction.

2. Submission of accurate mileage reports for personal reimbursement.
3. Use of agency vehicles, computers, office supplies, etc.
4. Accountable for inventory items used in course of work with Members.

Job Type: Full-time

Pay: $40,000.00 - $44,000.00 per year
