Position Summary: The incumbent will provide strength-based support to those seeking recovery or in recovery from substance use disorder. Those in this position will assist the individual in self-directed recovery while providing lived experience and expertise to support the change. This position requires the incumbent to exercise strong boundaries while remaining comfortable with self-disclosure. The position requires strong ties to the community and experience in navigating systems.
Education and Training: High School Diploma or equivalency preferred.
Experience and Work Knowledge: Minimum of 1-year experience working in human services preferred.
Physical Demands: No extraordinary physical demands.
Essential Duties:
- Provide environments to encourage self-advocacy, educational opportunities, referral to services, peer, and recovery support, and wrap around services as indicated.
- Serves as a personal guide for those seeking or supporting their recovery
- Appropriately represents the agency in community events, meetings, and forums to increase community awareness and participation.
- Orients residents to the facility, neighborhood, and local community resources to foster stability.
- Coordinates with the Vocational Education Rehabilitation Specialist to develop and implement programming in support of the acquisition of daily living skills, community living, and work readiness skills to support recovery.
- Assist the Housing staff on special projects as assigned.
- Enhance coordination of policies, procedures, and services across all housing programs through collaboration with the Housing management team.
- Provide written report and evaluation on client social-emotional presentation, behaviors, and contributions to the recovery community as per program requirements.
- Assist the Housing staff to develop and integrate new programs while meeting organizational and grant objectives to assist residents in achieving the greatest outcomes.
- Implement and strategically guide Best Practices in homeless service delivery, including Critical Time Intervention and Trauma-Informed Care.
- Evaluate crisis situations on an individual basis and effectively communicate all concerns and situations with the Housing staff
- Work collaboratively with each SJATRC team and outside provider team to identify appropriate services
- Bolster substance treatment services provided with SJATRC and provide assistance with residents concerning growth and sobriety.
- Promote the philosophy of social rehabilitation as it applies to residential treatment.
- Monitors the progress of each client and documents progress/regression in the client files. Meet with residents individually or in group session as indicated.
- Assist with discharge planning; and assist clients with developing and maintaining wrap around services.
- Develop and maintain good working relationship with client referral sources and other related mental health, substance abuse, and community service programs.
- Ensures program is in compliance with all agency, clinical, fiscal standards and licensure requirements.
- All other duties as assigned by agency management.
