Overview:
We’re looking for an experienced Patient Care Coordinator Non - Clinical to join our team.
You will report directly to the Clinical Director or Administrator. You will be responsible for administering of day to day operations and coordination of schedules of all team members.
- Scheduling out visits requested by Registered Nurses, Social Worker and Chaplains, and Aides as well as patient PT and wound care visits.
- Ensuring staff are completing visits in a timely manner
- Serving as the scribe and facilitator for interdisciplinary team meetings
- Order and coordinate delivery of DME and supplies
- Ensuring that Medicare patients have eligibility and verifying commercial insurance
About You:
- High School Diploma or GED
- Must have Kinnser/Wellsky experience (test will be performed at interview)
- Three to Five years’ experience in an administrative role
- One year administrative experience in a hospice setting
- Must be able to effectively operate office equipment to include computer and tablet
- Must be able to demonstrate ability by performing a variety of tasks without frustration when changing from one task to another
- Knowledge of terminally ill patients and their families along with understanding of hospice concept
