Description

StarCare Specialty Health System (StarCare) seeks to hire a Veteran Health Care Navigator to join the VetStar Team in Amarillo! Work is performed under the minimal supervision of the Veteran Programs Director with latitude for the use of initiative and independent judgment.

 

Health Care Navigator Responsibilities:

  • The Health Care Navigator (navigator) provides case management services that lowers barriers to appropriate healthcare for SSVF-eligible veteran families. This includes connecting veterans to Department of Veterans Affairs Health System (VAHS) health care benefits or community health care services where veterans are not eligible for VAHS care.
  • The navigator work closely with the veteran’s health care team to ensure timely and efficient access to health resources needed for housing stability.

 

Essential Duties:

The navigator will; participate in SSVF case conference and program quality improvement and quality assurance activities; act as a liaison between the SSVF grantee and the VA or community medical clinic and other healthcare providers, coordinating care for a population of veterans with health needs who require assistance accessing health care services or adhering to health care plans; work closely with the Veteran’s assigned multidisciplinary team, including medical, nursing and administrative specialists, and case management personnel; work within SSVF team to provide timely, appropriate, Veteran centered services in an equitable manner; work collaboratively with healthcare team and Veteran to identify and address system challenges for enhanced care coordination as needed; as necessary, provide coverage to screen and enroll eligible low-income veteran families in housing stability crises; outreach to the community and develop substantive networks among Department of Veteran Affairs, veteran service, homeless prevention, supportive service, and housing agencies that advance the interests of homeless veteran families; advocate for homeless veteran families and this program with a vision towards ending veteran homelessness in the community; complete timely and accurate reports and documentation necessary for program compliance; perform other duties as assigned by the veterans service coordinator.

 

Schedule: This is a full-time/non-exempt position working Monday - Friday, 8am - 5pm.  Flexibility is required to meet the needs of the program.

Qualifications


Qualifications:

  • Bachelor’s degree from an accredited four (4) year college or university with major course work in Human Services or a related field is preferred.
  • Some college and/or high school diploma will be considered.
  • A veteran or member of the U.S. Armed Forces (any branch), experience as a veteran caregiver, or two (2) years’ experience with state or federal military and veteran programs is required.

 

Additional Requirements:

  • Skill in operating standard office machines (telephone phone, fax, copier, calculator, etc.) and personal computer with Microsoft Office (Excel, Word, email, etc.).
  • Some local travel is required. Must have reliable transportation. Mileage reimbursed at state approved rate.
  • Must pass a pre-hire drug screen and criminal background check.
  • Must have a Texas driver's license and be insurable under the Agency's insurance.
