Naturopathic Medicine Primary Care Clinic seeking qualified patient care coordinator to join our team of providers and staff. Nestled in the heart of the Green Mountains of Vermont, it is an ideal location for someone who loves outdoor adventures!

We are looking for an enthusiastic, self-motivated and organized individual to fill this role. The Patient Care Coordinator is a vital member of the Stowe Natural Family Wellness team, collaborating closely with patients, their families/caregivers, and communities to identify and achieve health goals. Working in tandem with practitioners at SNFW, the coordinator assists with the creation and implementation of comprehensive care plans to support patients in achieving optimal health outcomes. This role is pivotal in providing care coordination, transitional care interventions, and connecting patients with essential community resources.

Key Responsibilities:

  • Patient-Centered Care: Engage with patients and their support systems to understand their unique health needs and goals.
  • Compliance with Vermont Blueprint for Health: Collaborate with the Vermont Blueprint for Health program to ensure adherence to program guidelines and regulations. Utilize Electronic Medical Records (EMR) systems to generate and analyze reports, ensuring that the organization meets the requirements of the program.
  • Patient Follow-Up: Conduct timely follow-ups with patients after visits to the emergency department, specialists, and after urgent or acute care visits. Assess patient progress, address any concerns or issues, and provide necessary support and guidance in collaboration with providers.
  • Care Plan Development: Collaborate with healthcare professionals to create personalized care plans that address the holistic health of patients.
  • Community Resource Systems: Connect patients with relevant community resources such as social services, support groups, and financial assistance programs. Have a comprehensive understanding of family systems, community resources, insurance processes, and legal/regulatory frameworks impacting healthcare outcomes.
  • Therapeutic Relationship Building: Establish and nurture trusting relationships with patients, providing them with emotional support, encouragement, and health education. Educate patients and their families about health management strategies, treatment options, and available support services.
  • Participation in Meetings: Attend meetings for the Lamoille County Community Health Team to collaborate on community health initiatives, share updates, and contribute to strategic planning. Participate in meetings related to the Vermont Blueprint for Health, providing insights, updates, and ensuring alignment with program goals and objectives.
  • Reception Duties Coverage: Cover reception duties as needed, including answering phones, scheduling appointments, and assisting patients with administrative tasks. Maintain a welcoming and organized reception area to facilitate smooth patient experiences.

Qualifications:

  • Prior experience in care coordination, social work, customer service, case management, or healthcare advocacy preferred.
  • Knowledge of healthcare system dynamics, community resources, insurance procedures, and regulations.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work collaboratively in a multidisciplinary team and independently when necessary.
  • Empathy, compassion, and a patient-centered approach to healthcare delivery.
