Our clinicians have a long history of collaboration with area physicians to help their clients get well after premature birth, complicated orthopedic surgeries and chronic illnesses. Summary/Objective

As part of a vibrant team, delivers aid, verbal cueing, reminding, and direct hands-on care for the elderly, convalescents, or persons with disabilities within the scope of practice to SummitWest Care clients.
Essential Functions

  • Assesses client and formulates a plan of care that includes social, emotional, physical, and spiritual needs.
  • Establishes and executes care plan consistent with physician orders, client choice and regulations.
  • Assures physicians are notified in a timely manner of changes in client status and physician’s orders are obtained as needed for changes to the plan of care.
  • Provides teaching and instruction to client/caregiver to promote physical and emotional health.
  • Contributes to the coordination of services through effective and timely communication with other members of the health care team.
  • Maintains the caseload and agency productivity standards and participate in the on-call duty rotation.
  • Attends required staff meetings and case conferences.
  • Supervises care being provided by LPN and Home Health Aides to assure that clients are receiving appropriate and quality care and regulatory guidelines are met.
  • Assumes responsibility for the delegation of nursing tasks and for the supervision of delegated tasks by other health care workers.
  • Assures client rights are protected and that all clients/caregivers are made aware of the client’s rights and responsibilities.
  • Initiates emergency procedures as appropriate.
  • Clinical and administrative documentation adheres to agency, professional, and regulatory guidelines for timeliness and content.
  • Performs other duties within scope of practice, as assigned by Clinical Manager
  • May require extended staff schedules.
  • Other duties as assigned.

Supervisory Responsibility
This position has no supervisory responsibilities.
Required Education and Experience * Graduate of an approved School of Nursing

  • Currently licensed and in good standing in the State of Colorado
  • Previous home care experience preferred.

Language Skills
1. Ability to read, write, and present information one-on-one and to small groups in English is required.

Work Environment: The following work environment characteristics represent those an employee typically encounters while performing the essential functions of this position.

This job operates 60% of the time indoors; 40% of the time outdoors. Occasional exposure to physically and/or verbally aggressive clients. Noise level is generally moderate. Job tasks involve exposure to blood, body fluids, and/or tissue.
Physical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee will stand 50% of the time, sit 10% of the time, and walk 40% of the time. The employee frequently is required to climb, balance, stoop, kneel, crouch, crawl, and reach with arms.

The employee must be able to grasp, pinch, and feel with hands and fingers and be able to lift, carry, push, pull, or otherwise move objects up to 50 lbs. frequently and up to 100 lbs. occasionally. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. Specific hearing abilities required by this job include ability to perceive the nature of sounds at normal speaking levels with or without correction. Must be able to receive detailed information through oral communication, and to make the discriminations in sound.
Position Type/Expected Hours of Work
This is a full-time position typically Monday through Friday. Occasional evening and weekend work may be required as job duties demand.
