Sunbelt is seeking a Registered Nurse for a Home Health contract position with a local client near Cleveland, OH. Employment Type: Full-time, 13 week contract with potential to extend
Works Hours: 8:00AM-5:00PM, 40 hours per week, some on call and weekends required
Responsibilities:
- Utilize expert knowledge of OASIS-CAssessment Documentation Start of Cares/Admissions, Recertifications, Resumptions of Care, Transfers, and Discharges
- Complete accurate and detailed OASIS-C Documentation within a 24-48 hour turnaround time
- Case manage at least 5-7 patients per day and 25-30 patients per week
- Scheduled visits will include, but are not limited to:
- Providing wound care
- IV administration and teaching
- Cardiac program follow up and intervention
- Travel in theCleveland, OHarea
Experience:
- At least one year of Home Health experience within the last year required
- In depth knowledge and in-field experience of OASIS-C required
- Start of Care/Admissions experience required
- Experience case managing at least 5-7 patients per day and 25-30 per week
Education:
- Associates Degree of Nursing or Bachelors Degree of Nursing
- OhioRegistered Nurse License
- American Heart Association Basic Life Support
