Sunshine Home Health Care
Our Home Health Care Facility provides health care in the home to address recovery from illness, aging in place, management of chronic disease to post-surgical care and recovery from inpatient stays. Responsibilities of the Home Health PTA
- Reviews patient history and appropriate data prior to making assigned visits.
- Follows through on assigned patients and their plans of care as outlined by a Physical Therapist.
- Evaluates clients’ response to treatment and notifies the Physical Therapist when the plan of care may need to be changed.
- Keeps Physical Therapist updated regarding care provided and patient progress, decline or plateau.
- Assumes responsibility and accountability for care provided under the direction of a Physical Therapist.
- Identifies education needs of the patient/caregivers and addresses them as outlined in the plan of care. Assesses and documents learning and communication needs of client, family and or caregivers.
- Incorporates teaching interventions into visits as directed on the plan of care.
- Documents all treatments, procedures, education and patient response according to SHHC guidelines.
- Assures all visit documentation is completed the day of the visit.
Qualifications of the Home Health PTA
Education/Experience
- Graduate of accredited Physical Therapy Assistant program. Recent home health experience desirable.
- 1 - 2 years recent hospital, subacute or rehabilitation facility experience in a medical surgical area.
- Other relevant clinical experience in a community-based facility may be substituted.
Certificates/Licenses:
- Current Physical Therapy Assistant license in good standing
- Current driver’s license
- Current CPR certification
