FLSA Status: Non-Exempt
Availability: Day Shift (9 am - 5 pm/Monday-Friday)
Summary: The Case mMnager is to ensure continuity of care for the consumers. The Evening Case Manager will be responsible for executing and developing great relationships with staff and family, and also act as the liaison between the Support Coordinating Organizations/Behavior Specialists and/or any other external stakeholders to ensure that all individuals receive the necessary support and services they require and desire while apart of the agency’s program. You must be familiar with Charting the LifeCourse, In-Home and Community Supports, Community Participation Supports, Companion and Job Supports, Day Program. You must also have the ability to convert units and hours for budgeting.

Daily
Managing Clock in Clock outs
Scheduling
Reaching out to families to update them about attendance issues
Contacting staff regarding attendance concerns
Coordinating between Recruitment and Program Coordination departments
Weekly
Hold Information Sessions (Monday & Wednesday Evenings)
Review Progress Notes
Staff Updates to include their progress & progress notes
DSP Lead meetings to discuss DSP progress/concerns
Recruitment - Reach out to families/CLA /LifeSharing staffing updates
Follow Up on applications and requirements
Follow up with family 2nd week and then monthly
Identifying Resources, volunteering, and Activities based on ISP(opportunities to socialize - group oriented - the rec philly, clubs, special olympics, classes, special interest groups)
New cases (24/7 and/or 12 shifts daily - weekly check-ins until family is satisfied with staff. For example, Check-ins weekly for a month and then have the family decide if they want to continue every other week. Gain insight regarding family members' perspectives). Another example, the agency's discretion if we should continue to complete weekly follow-ups
Monthly
Virtual Wellness Checks with families/individuals (Family Resources, Providing Progress regarding Supports, Obtaining feedback from family)
Monthly progress check-in with staff, progress notes, advocacy, attendance
High school diploma and three to five years and/or related experience required. Bachelor's degree (B. A.) from a four-year college or university preferred.
Other Requirements, Skills, Knowledge and Abilities:
Must be able to successfully pass a criminal background check and health screening.
Required to maintain a valid driver’s license.
Good communication skills (verbal and written), interpersonal, and organizational skills required.
Must remain abreast of industry regulations and best practices.
Must complete any trainings and/or other requirement determined by the Agency’s regulatory agencies or required by the Agency.
Position requires employee to demonstrate s desire to assist all employees with achieving their potential.
Must be able to monitor and access one’s personal performance and the performance other individuals, or organizations to make improvements or take corrective action.
Possess a revenue-oriented mind-set
Ability to travel to include commuting to the office
Knowledge of the healthcare industry as it relates to the qualifications of support being provided
Manage your mileage reports and office laptop, if applicable
You will have to use your internet and electricity, if applicable
Experience: Must have experience with caseload management, typing, reading, writing and interpreting emails, and motivating staff. We are especially looking for someone who has soft skills and can take positive approaches to resolving tasks. A positive attitude is a must.

Salary: $41,000 to 47,000 per year
